These tools are NOT diagnostic tools and are not a substitute for clinical evaluation. Only a trained professional can diagnose a disorder.

Screening Awareness Event Registration - Facilitators use this form to register an upcoming event and request resources.
Participant Feedback Form - A short survey for participants to provide feedback after completing a screening tool.
Facilitator Feedback Form - Facilitators use this form to provide feedback on program resources, strengths, and challenges.
Event Tracking Form - Facilitators use this form to track the number of participants and on-site screenings completed for each awareness event offered.
