Restorative Care Program – Western Long Term Care Home
Patients following an acute care episode or chronic condition which delays discharge or prevents an individual from returning or remaining home at their pre-admission level of care may benefit from Restorative Care. Restorative Care can help patients regain the functional skills necessary for day-to-day living, mobility, and independence to return/remain home.
Restorative Care is a slow-paced program; meaning fewer hours of therapy a day complimented by consistent encouragement to participate in activities of daily living.
A 15 bed Restorative Care program is available at the Western Long Term Care Home and offers individuals the opportunity to improve and go home after an illness or surgery in a positive environment.

Objectives:
To discharge individuals back to their previous living arrangement at an optimal level of functioning
To reduce use of acute care beds for alternate level of care therefore improving access to appropriate acute care beds and reducing overflow
Eligibility:
· Be medically and physically stable; acute care issues have been identified and addressed by the current care team
· Have functional and attainable goals
· Specific goals of care must be established in acute care; patients must have objectives to work on while in the program
· Be motivated and agreeable to actively participate in goal setting
· Cognitively be able to learn new tasks and retain therapy instruction
· Be able to tolerate sitting up in bedside chair for at least 3 meals a day
· Aged 65 and older (Individuals below this age may be looked at on a case-by-case basis with priority given to those aged 65 and greater)
The Restorative Care facilitator will communicate with the care team or patient/client/resident if an individual does not meet eligibility criteria.
Cost:
There is no cost for board and lodging. There may be costs associated with attendance at outside functions.
Consideration for Restorative Care:
All individuals who meet eligibility criteria will be reviewed by the Restorative Care facilitator for suitability and their readiness for transfer to the Restorative Care program.
