Community Health Needs and Resources Assessment Survey Available on Paper

Over 400 online surveys have been completed in Western Health’s Community Health Needs and Resources Assessment since January 1.  Residents who wish to complete the survey on paper now have that option. The survey will be mailed to people who request to complete it on paper.

The survey asks questions about health and community services in the region. The Community Health Needs and Resources Assessment will also include focus groups, health status statistics, and community resource listings. The results of the assessment will be used to help us plan our programs and services.

“The uptake has been great so far, but we’ve received some feedback from people who do not have access to do the survey online, and we want to make sure the survey is available to everyone who wants to complete it,” says Darlene Welsh, Regional Director of Planning and Research. “If you wish to participate in the survey on paper, either because you do not have access to do so online, or if your preference is to do it on paper, please contact us and we will mail you a survey that you can return by mail.”

Residents can call 709-637-5000 ext. 6419 to request the survey to be mailed to them. The survey is also still available on Western Health’s website until February 29, 2016.
